The SoutienTMC project will be led by<strong> Ariane Girard<\/strong>, LHS FRQS Young Leader and professor at the School of Nursing, Faculty of Medicine and Health Sciences, Universit\u00e9 de Sherbrooke, and Jo\u00eblle Bernard-Hamel, patient partner. <strong>Catherine Hudon, Catherine Wilhelmy and Yessica-Haydee Gomez<\/strong>, respectively co-scientific director, co-patient partner and coordinator of the SSA-RUISSS Universit\u00e9 de Sherbrooke team of the Unit\u00e9 de soutien SSA Qu\u00e9bec, will ensure that the project is aligned with the priorities of the Minist\u00e8re de la Sant\u00e9 et des Services sociaux (MSSS) and the Unit\u00e9, and that it fosters partnership work and the promotion of the patient perspective.<\/p>\n\n\n\n<div class=\"wp-block-buttons class=\"wp-block-heading\"><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The SSA-RUISSS Universit\u00e9 de Sherbrooke team is launching a project to improve practices and the trajectory of services for people living with anxiety or depression <strong>while they are receiving primary care outreach services.<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Primary care services are provided by family medicine groups, general social services and primary mental health teams of the CLSCs of the CISSS\/CIUSSS, community organizations and private clinics.<\/p>\n\n\n\n<figure class=\"wp-block-paragraph\"><\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><h3>Issue<\/h3><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">In the scope of this project, the SSA-RUISSS Universit\u00e9 de Sherbrooke team is considering the following problematic:<\/p>\n\n\n<div class=\"wp-block-list\">\n<ul class=\"wp-block-list\">\n<li>Primary care teams do not always have access to the clinical and organizational support required to ensure that patients receive the best interventions for their health and recovery needs;<\/li>\n\n\n\n<li>The service trajectory is not always clearly defined and there are redundancies in the steps, a lack of access to best practice knowledge that reflects the complexity of the practices, and difficulty accessing experts.<\/li>\n<\/ul>\n<\/div>\n\n\n<h3 class=\"wp-block-heading\"><h3>Hypothesis and objectives<\/h3><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">To address these issues, the team offers three hypotheses for improvement:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">1) Improve identification and assessment practices;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">2) Enhance clinical monitoring activities as part of care management to support trajectory alignment with patient needs;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">3) Improve continuity when a stage in the trajectory involves moving from one stakeholder to another within different organizational teams.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><h3>Participatory research approach of the <a href=\"https:\/\/ssaquebec.ca\/en\/news\/what-is-a-living-lab\/\">living laboratory<\/a> type<\/h3><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Within the framework of the SupportTMC project of the PATIenTS program, the team in charge adopts an innovation development approach centered on the needs of the users in their real environment to plan the improvement of the trajectory (consultations, co-design) and to aim at the empowerment of local actors in parallel with the research work.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This work is carried out in partnership with the five CISSS\/CIUSSS of the RUISSS Universit\u00e9 de Sherbrooke, a group of patient-clinician experts (advisory), the RUISSS steering committee, as well as local teams or change agents.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><h3>Impact Measurement<\/h3><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">As part of the SupportTMC project of the PATIenTS program, the project team identified items that could be measured to assess the impact of the project on each of the class=\"wp-block-heading\"><h3>PATIenTS Program<\/h3><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The Unit\u00e9 de soutien SSA Qu\u00e9bec is launching the <a href=\"https:\/\/ssaquebec.ca\/en\/news\/new-patients-program-4-tangible-projects-to-improve-life-trajectories-in-quebec\/\">Life Pathway Improvement and Innovation Partnerships in Transition to Sustainable Health (PATIenTS program)<\/a> in collaboration with the four integrated university health and social services networks (RUISSSs). Through the PATIenTS program, the activation of the Qu\u00e9bec learning health system is being carried out in the RUISSS territories of Qu\u00e9bec through four tangible projects over a one-year
