Discutons-Sant\u00e9<\/p>\n\n\n<div class=\"wp-block-list\">\n<ul class=\"wp-block-list\">\n<li>Claude Richard<\/li>\n<\/ul>\n<\/div>\n\n<\/div>\n\n                    \n\n\n\n<h3 class=\"wp-block-heading\"><strong>Guichet d\u2019acc\u00e8s \u00e0 la Premi\u00e8re ligne (GAP)<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Launched in 2022 by the Ministry of Health and Social Services (MSSS), the Guichet d&#8217;acc\u00e8s \u00e0 la premi\u00e8re ligne (GAP) allows people without a family doctor to obtain a medical appointment or health service.<\/p>\n\n\n\n<figure class=\"wp-block-image size-large\"><img decoding=\"async\" src=\"https:\/\/ssaquebec.ca\/wp-content\/uploads\/2023\/03\/trajectoire_GAP_RUISSSUdeM_ProgPatients-1024x314.png\" alt=\"\" class=\"wp-image-15693\"\/><\/figure>\n\n\n\n<h3 class=\"wp-block-heading\"><h3>GAP care and service trajectory<\/h3><\/h3>\n\n\n\n<figure class=\"wp-block-image size-full\"><img loading=\"lazy\" decoding=\"async\" width=\"900\" height=\"301\" src=\"https:\/\/ssaquebec.ca\/wp-content\/uploads\/2023\/03\/trajectoire_GAP_RUISSSUdeM_ProgPatients_2-e1679422396191.png\" alt=\"\" class=\"wp-image-15699\" srcset=\"https:\/\/ssaquebec.ca\/wp-content\/uploads\/2023\/03\/trajectoire_GAP_RUISSSUdeM_ProgPatients_2-e1679422396191.png 900w, https:\/\/ssaquebec.ca\/wp-content\/uploads\/2023\/03\/trajectoire_GAP_RUISSSUdeM_ProgPatients_2-e1679422396191-300x100.png 300w, https:\/\/ssaquebec.ca\/wp-content\/uploads\/2023\/03\/trajectoire_GAP_RUISSSUdeM_ProgPatients_2-e1679422396191-768x257.png 768w\" sizes=\"auto, (max-width: 900px) 100vw, 900px\" \/><\/figure>\n\n\n\n<p class=\"wp-block-paragraph\"><h3>Issues<\/h3><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">As part of this project, the SSA-RUISSS Universit\u00e9 de Montr\u00e9al team identified the following issues:<\/p>\n\n\n<div class=\"wp-block-list\">\n<ul class=\"wp-block-list\">\n<li>Asking patients to limit their medical consultation to the discussion of a single problem leads to frustration, especially for patients with multiple problems, thus negatively affecting the patient experience<\/li>\n\n\n\n<li>Encountering orphaned patients who have complex needs and\/or require more than one visit to stabilize their condition places great pressure on caregivers<\/li>\n\n\n\n<li>There are no mechanisms in the GAP to document the post-initial consultation trajectory and follow-ups required for stabilization of the patient&#8217;s condition<\/li>\n<\/ul>\n<\/div>\n\n\n<h3 class=\"wp-block-heading\"><h3>Objectives<\/h3><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">To address these issues, the team aims to achieve two goals:<\/p>\n\n\n<div class=\"wp-block-list\">\n<ul class=\"wp-block-list\">\n<li>Improve communication between patients and caregivers in order to :<div class=\"wp-block-list\">\n<ul class=\"wp-block-list\">\n<li>Makeconsultations more efficient<\/li>\n\n\n\n<li>Improve the patient experience<\/li>\n\n\n\n<li>Reduce pressure on healthcare staff<\/li>\n\n\n\n<li>Document the necessary actions following an initial medical consultation referred by the GAP<\/li>\n<\/ul>\n<\/div><\/li>\n<\/ul>\n<\/div>\n\n\n<p class=\"wp-block-paragraph\"><strong>How can we continuously improve the trajectory for people without a family physician using the Primary Care Access Portal?<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">As part of its PATIenTS project, the SSA-RUISSS Universit\u00e9 de Montr\u00e9al team proposes two concrete improvements:<\/p>\n\n\n<div class=\"wp-block-list\">\n<ol class=\"wp-block-list\">\n<li><strong>Integrate the Medical Appointment Preparation Form (FPRM) (DiscutonsSant\u00e9.ca &#8211; Capsana) into the GAP trajectory to<\/strong><div class=\"wp-block-list\">\n<ul class=\"wp-block-list\">\n<li>Helping patients<div class=\"wp-block-list\">\n<ul class=\"wp-block-list\">\n<li>Actively preparing patients for the consultation offered by GAP helps to improve their experience of care<\/li>\n\n\n\n<li>It is important to identify the priority issue for discussion<\/li>\n<\/ul>\n<\/div><\/li>\n\n\n\n<li>Supporting the health care staff<div class=\"wp-block-list\">\n<ul class=\"wp-block-list\">\n<li>By having access to the FPRM form prior to the consultation, caregivers will be better able to plan the consultation and adjust quickly to the demand<\/li>\n<\/ul>\n<\/div><\/li>\n\n\n\n<li>Promote a patient-professional partnership<\/li>\n\n\n\n<li>Improve communication at the time of consultation<\/li>\n\n\n\n<li>Contribute to the efficiency of the meeting<\/li>\n<\/ul>\n<\/div><\/li>\n\n\n\n<li><strong>Document the pathway after the initial medical consultation<\/strong><div class=\"wp-block-list\">\n<ul class=\"wp-block-list\">\n<li>To better anticipate the nature and duration of follow-ups required by the patient&#8217;s health status, particularly in cases where stabilization of chronic or complex problems is targeted<\/li>\n\n\n\n<li>Soliciting collaboration with a subset of patients with complex or chronic problems who have been referred by the GAP to:<div class=\"wp-block-list\">\n<ul class=\"wp-block-list\">\n<li>Access clinical-administrative data from the electronic medical record (EMR)<\/li>\n\n\n\n<li>Access the FRPM form (long version) to reflect patient experience (preparation history, notes and comments)<\/li>\n<\/ul>\n<\/div><\/li>\n\n\n\n<li>In order to better anticipate the actual workload of the nursing staff:<div class=\"wp-block-list\">\n<ul class=\"wp-block-list\">\n<li>Conclude the episode of care for which the orphan patients were referred to them by the GAP<\/li>\n\n\n\n<li>Manage the necessary resources<\/li>\n<\/ul>\n<\/div><\/li>\n<\/ul>\n<\/div><\/li>\n<\/ol>\n<\/div>\n\n\n<figure class=\"wp-block-image size-large\"><img decoding=\"async\" src=\"https:\/\/ssaquebec.ca\/wp-content\/uploads\/2023\/03\/FPRM_RUISSSUdeM_ProgPatients-1024x948.png\" alt=\"\" class=\"wp-image-15695\"\/><\/figure>\n\n\n\n<p class=\"wp-block-paragraph\"><\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><h3>Impact Measurement<\/h3><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">As part of the RUISSS PATIenTS project, the project team identified elements that could be measured to assess the impact of the project on each of the elements of the quintuple aim. Through the PATIenTS program, the activation of the Qu\u00e9bec&nbsp;<a href=\"https:\/\/ssaquebec.ca\/en\/the-unit\/learning-health-system\/\">learning health system<\/a>&nbsp;is being carried out in
