Context
The life history questionnaire is a tool used to support the transition of older adults into long-term care facilities (CHSLD) and to personalize care. The aim of this study was to describe the perceptions of professionals and managers regarding the use of the life history questionnaire in the Côte-Nord region and to understand the factors influencing its implementation in this context.
Method
We conducted a qualitative study. Interviews were analyzed using a descriptive-interpretive approach.
Results
Social workers play a key role in completing the questionnaire and discussing its use with other staff members, who value the tool for personalizing care. Barriers to its use include high staff turnover and the difficulty of fostering a culture shift. Facilitators include institutional support, access to completed questionnaires, team stability, and interprofessional and inter-institutional collaboration.
Conclusion
The life history questionnaire is useful for facilitating the transition process, and adopting it prior to admission could enhance its value.
