- PREMs and PROMs Toolkit: patient-reported indicators of health outcomes and experiences
Patient-reported experience measures (PREMs) are self-reported measures that provide insight into how patients experience care.
The person receiving care is the expert in their own life with their own health condition. They must be included as a partner in care to ensure optimal management of their condition according to their values and preferences. To promote person-centred care, many health and social service professionals invite patients to share their views about their care experience. The use of patient questionnaires is necessary to gather this information. The survey of PREMs and PROMs continues after care, in the home.
