The Unité de soutien SSA Québec kicked off the second cohort of the LHS Incubator on September 9 and 10, 2026, in Quebec City. This was an opportunity for the selected teams to get to know one another, participate in the first three workshops on the topics of partnership, project identity, as well as equity, diversity, and inclusion, and network with potential partners.
Since 2025, the LHS Incubator has been providing intensive support to project teams within the health and social services network. It aims to help participating teams integrate the concepts of a learning health system into their projects. This is a collaborative, co-constructed approach carried out in partnership. The result is the mobilization of knowledge and guidance for teams in incorporating impact measurement. The experience can be summarized as follows:

Chaudière-Appalaches
The project aims to structure the innovation function at Santé Québec Chaudière-Appalaches into a learning organization. To achieve this, the team hopes to establish an Innovation Hub by building on both existing initiatives and the organization’s emerging needs in order to strengthen its coherence and scope.
The project will help structure the organizational learning cycle (capture, analysis, support, scaling, and transfer) through governance mechanisms, shared decision-making, and collective learning to increase impact and value creation within the institution. The team aims to increase its capacity to measure impact, create value, and continuously improve prioritized projects.

The project aims to centralize food production in the Baie-Comeau and Haute-Côte-Nord regions in order to:
- Improve operational performance by 30%
- Concentrate expertise in the central kitchen to mitigate the effects of the labor shortage
- Enhancing Food Safety Through Quality Control of Textured Foods for Clients with Dysphagia

Gaspésie
This project aims to improve the accessibility, efficiency, and quality of primary-care adult mental health services by developing an integrated, person-centered care pathway.
Inspired by the principles of the Quebec Mental Health Program ( PQPTM) and the philosophy of palliative care, it offers a patient-centered clinical approach, a care navigation system that promotes continuity of care, a pilot pathway with an in-house program, a structured partnership with a community organization, and rapid-access group services.
The goal is to provide the right service at the right time, while minimizing disruptions to the care pathway and maintaining the quality of the clinical encounter. This initial six-month phase will allow us to test and refine this model in order to support its gradual rollout, particularly by adding new partners, service pathways, and interventions tailored to the needs of the population.

Montréal
The project aims to establish peer-to-peer support to foster knowledge transfer and mobilization by applying the principles of a learning health system.
Santé Québec Centre-Ouest de l’île de Montréal (CCOMTL) aims to develop its teams’ skills in the areas of knowledge transfer and mobilization, implementation science and scaling up, as well as mentoring the next generation of professionals.
The organization is therefore implementing a project to integrate the principles of experiential-socioconstructivist learning into its daily operations. These integrations are intentionally geared toward the implementation of new practices, which are then documented and evaluated so that they can be consolidated into a structured and transferable service offering.
This process will also lead to the development of a structured and scalable peer support model that can be implemented organization-wide.
